01. Completing referral
Call us or complete the enquiry form in the website
02. Plan for care and services
One of our coordinator team members will be in touch to discuss the support services you need to create your personalised care plan. They can either complete this over the phone, online or in person in the comfort of your own home.
03. Assessment phase
Required clinical assessments will be completed by our clinical team in consultation with you to tailor quality services for you.
04. Commencing services
We aim to commence services as soon as possible or a weeks time from your first contact with us.
Please note: complex care services might take a little bit longer as we would like to ensure we set up appropriate level of care you require. However, we always endeavour to provide the support you need when you need it.

What qualifications do Adore Healthcare staff have?
Our team comprises qualified professionals, including registered nurses with current AHPRA registration, NDIS-trained support coordinators, care workers with Certificate III or IV in Individual Support or equivalent disability qualifications, and all staff hold current NDIS Worker Screening Clearances, First Aid/CPR certification, and receive ongoing professional development training.

Person-centered care means putting you at the center of all decisions about your support. We work with you (not for you) to understand your goals, preferences, strengths, and needs. Your care plan is designed around your individual circumstances and aspirations, giving you choice and control over how services are delivered and who provides them.
