What types of life transitions can be supported?

We support all significant life changes including moving to independent or supported living, starting employment or education, transitioning from school to adult life, relationship changes (moving in with partner, marriage, separation), becoming a parent, aging and changing care needs, transitioning between different accommodation types, adjusting after major health events, and any other major life change where you need support to adjust and succeed.

How long does transition support last?

Duration varies based on the complexity of the transition and your individual needs. Some transitions require intensive support for 3-6 months, others need ongoing but gradually reducing support over 12-24 months. For example, moving home might need intensive support for 2-3 months while you settle in, then periodic check-ins. Starting a job might need several months of on-the-job coaching. We work with you to determine appropriate support timeframes.

Is transition support only for young people?

No! While transitioning from school to adulthood is a common transition, participants of all ages face significant life changes. We support seniors adjusting to aging and changing needs, middle-aged participants changing living arrangements or careers, and anyone at any life stage navigating major changes. Transitions happen throughout life, and support is available regardless of age.

Can family members be involved in transition support?

Absolutely, and we encourage it! Family involvement is often vital for successful transitions. We can work with families to prepare them for changes (e.g., their loved one moving out), coordinate between family support and professional support, include families in planning and decision-making, and help establish new roles and boundaries that support independence while maintaining family connection. The level of family involvement is based on your preferences.

What if my transition doesn't go as planned?

Not all transitions go smoothly, and that's okay. If challenges arise, we increase support intensity, reassess the plan and make adjustments, identify what's not working and problem-solve together, provide additional skill development in challenging areas, and ensure you feel supported during setbacks. Transitions rarely follow a perfectly linear path, and flexibility is built into our approach.

How is transition support different from support coordination?

Support coordination helps you implement your entire NDIS plan and connect with all services. Transition support is more specific and intensive, focusing on building skills and providing hands-on support for a particular life change. You might have a support coordinator managing your overall plan AND transition support workers helping with a specific transition. They can work together to ensure your transition succeeds.

Can transition support help with finding a job?

Yes! Employment transitions are a major focus of transition support services. We help with job searching and applications, interview preparation, workplace orientation, initial on-the-job support, developing workplace skills, liaison with employers, and adjustment to employment routines. This is separate from Disability Employment Services (DES) but can work alongside them.

What if I'm nervous about the transition?

It's completely normal to feel nervous about major life changes! Our approach acknowledges these feelings while building your confidence. We work at your pace, break transitions into manageable steps, celebrate small victories, provide emotional support and encouragement, connect you to counseling if needed, and ensure you never feel rushed or pressured. Many participants feel anxious at the start but gain confidence as they see their progress.

Is transition support funded separately in my NDIS plan?

Transition support is typically funded under Capacity Building - Assistance with Life Stage, Transitions and Supports. Your plan should specifically include this funding if you've identified a major life transition. If you're facing a transition but don't have this funding, you may need a plan review to have it added. Some transition activities might also be funded under Core Supports depending on the specific task.

Can transition support be used for emergency or unexpected changes?

While transition support is usually planned for anticipated changes, we recognize unexpected life events occur (sudden family changes, emergency accommodation moves, unexpected health issues requiring adjustment). If you have transition support funding in your plan, it can be mobilized for unexpected transitions. If not, your support coordinator may help arrange crisis support or pursue a plan variation for additional funding.
